Strategies, Partnerships, and Policies for Supporting
Post-Incarceration Transitions

Staff conducts regular follow-ups at 30, 60, and 90 days, as well as at six months and 12 months.

Participants are encouraged to engage in community activities such as service organizations, churches, projects, and support groups.
